What to Bring
You can save time in advance by filling out and submitting your patient registration forms online. If you do not submit them on our website, please arrive a few minutes early to your appointment to complete any remaining paperwork. We ask that you bring the following to the consultation:
- A list of current medications
- Referral slips, if applicable
- Valid method of payment
- Dental or medical insurance cards
- A list of questions you have for the dentist
Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation appointment.
What to Expect
The first dental appointment often involves an oral exam and professional teeth cleaning. We will examine your mouth, look for any abnormalities, and clean your teeth. If you require sealants, fillings, crowns, or other same-day procedures, we may perform treatment on the same day as the consultation. At the end of your visit, we will schedule any future appointments and provide you with any instructions or information you may need.
Dental X-rays
During most consultations and at regular cleanings, a dental hygienist will take X-rays of your mouth to examine your teeth and jaws.

Symphony Dental Studio accepts a variety of payment methods, insurance plans, and financing options to make excellent dental treatment more accessible. If you have questions about payments to your account, please call our office. We recommend contacting your insurance provider directly before your appointment to find out the details of your coverage. Our office accepts the following payment methods:

Wells Fargo Health Advantage®
If you’re interested in financing your dental treatment, you can apply for a Wells Fargo Health Advantage credit card. Once approved, you can use your credit card immediately and pay off your balance over time. There are monthly payment plans available upon approval, and it takes only a few minutes to apply.
